Both full-time and part-time applications will be considered for this role. The ideal candidate will be an independent prescriber, and have experience working in General Practice as a Clinical Pharmacist. The Clinical Pharmacist will act autonomously, within their professional scope of practice, providing care for patients face to face and via telephone consultations. The post-holder will operate within their own sphere of competence, and agreed acceptable limits of practice, to provide expert professional clinical advice to patients, carers, and colleagues. The successful candidate will use skills, knowledge and competencies, as a qualified Clinical Pharmacist, in order to be responsible and accountable for managing medications, and supporting the Prescription Team with queries and requests. About us Work as part of a multi-disciplinary team to deliver safe and high quality care. Supporting the Prescription Team (Pharmacy Technicians, Prescription Administrators, Dispensers) with medication queries and prescription requests. Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice. Supporting the clinical team with the management of long-term conditions and chronic disease. Requesting blood tests, blood pressure, and other monitoring as part of safe medicines management and drug monitoring File incoming clinical correspondence to the Practice, and action subsequent tasks or requests as appropriate. Participate in the care home ward rounds. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Ensure clinical systems kept up to date, recording and/or amending accurate details. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice. Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking. "}
