To provide effective caseload management and care to those patients over the age of 18 requiring nursing care in their own homes and other community settings working autonomously and independently. The post holder will provide holistic care, assessment of the new patient , clinical advice, treatment and support to those people who require additional support to remain in their own homes. The post holder will be required to work with the multidisciplinary team, gaining support and advice in the management of care.
