We are a busy and friendly training practice with 12 GP Partners, 3 salaried GP's, Registrars and ACP's, with excellent administrative support, based in rural North Devon with a patient cohort exceeding 19,300 patients. We are looking for an enthusiastic and motivated prescribing nurse who has the ability to lead, develop and support our nursing team and deliver patient care to high clinical and operational standards. You will ideally have qualifications to specialise in Diabetes and have knowledge and experience of dealing with other chronic diseases in addition to other Practice Nurse tasks. This role is also responsible for leading the annual flu campaign, including the planning and coordination of clinics, coordination of staff, to achieve high vaccination levels in our eligible patient population. It is essential that you have recent knowledge of working in Primary care and experience of leading and managing staff will be key to a successful application. The post is for 37.5 hours per week. The lead practice nurse will be responsible for a number of clinical areas such as health promotion and chronic disease management, specifically taking a lead in Diabetes. Working in accordance with all essential protocols and registration requirements, the post-holder will provide assessment, treatment, screening, health education services and advice to patients including all tasks normally undertaken by an experienced nurse. About us Management of Chronic Disease Provide a nurse led specialist management service for patients with diabetes, and in one or more of the following areas Sexual Health, Rheumatology, Hypertension. Drugs; domestic violence; child protection; vulnerable adults; social problems o Health Promotion Deliver opportunistic health promotion where appropriate, liaising and providing advice to other members of the practice team in terms of campaigns and promotion. Vaccinations and Immunisation (adult & child) Administer injections under an individualised prescription or PGD.
