Ensure appropriate records are maintained under the direction of the qualified practitioner. If so, we want to hear from you We are looking for an experienced and highly committed Support Worker for our Children and Young Peoples Crisis Response Team to support us in ensuring we deliver outstanding patient care. The service provides initial assessment of mental health needs, alongside intensive home treatment intervention to help promote community health and well-being . The team operate a 24/7 service and provides support to young people in a range of settings. Assist and promote independence with activities of daily living. Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff. When required support the service user to access other health services and ensure reasonable adjustments are implemented . Facilitate group work as directed by the qualified practitioner, to include; . Ensure appropriate records are maintained under the direction of the qualified practitioner.
