To provide a specialist clinical pharmacy service to clinics and specialist areas, with a particular focus on endocrine and diabetes To provide a daily clinical pharmacy service to designated wards to support the clinical pharmacy service. To provide clinical and financial management on Continuous Glucose Monitoring (CGM) and Hybrid Closed Loop treatments. To support the clinical management of patients who receive endocrine and diabetic treatments from the Trust, who out outside of the West Yorkshire Integrated Care System.
