To accept responsibility for designated caseload, working independently taking an active role in promoting independence and meeting the needs of the service user. To promote wellbeing, safe mobility, transfers and effective communication. To provide instruction, advice and intervention to service users and carers in line with the multi-disciplinary team guidelines. To assess, provide, fit and demonstrate use of a wide range of equipment to promote independence. To assess for and commission a range of adaptations in line with the multi-disciplinary team guidelines. To attend and contribute to team meetings and case discussions using effective communication. To maintain comprehensive and accurate records in line with employers polices. To promote health and wellbeing with all citizens. To undertake a range of nursing competencies, as per the AP competency framework. To operate under the guidance of clinicians and only within the scope of AP practice. Managerial and Leadership. To provide support to other Assistant Practitioners as appropriate. The base will be at Mary Potter Health Centre but patients will be seen in a variety of community settings.
