This is a fantastic opportunity to be part of an innovative service that is making a real impact on patient care, improving outcomes, and reducing strain on acute hospital services. Act as a key point of contact for the UCR service, managing urgent referrals, coordinating communication between clinicians, patients, and external services. Provide comprehensive administrative support, including diary management, scheduling patient visits, and maintaining accurate records of patient care. Prioritise and coordinate rapid response team activity, ensuring urgent cases are actioned swiftly and efficiently. Use electronic patient record systems to track and update patient information, referrals, and outcomes. Liaise with GPs, community services, and other healthcare professionals to ensure smooth communication and continuity of care.
