Your role as a Nurse / Allied Health Professional (AHP) Clinical Assessor within the adult division of Continuing Care will be key to ensuring that we are able to implement our above aim in a high-quality way. As a Nurse/AHP Clinical assessor, you will work under the guidance and support of the senior clinical team leads to implement the NHS Continuing Healthcare framework for those 18 years of age across Mid and South Essex. You will promote best practice, provide professional advice to providers, promote good relationships and be able to liaise with colleagues from other disciplines both internally and externally to the Integrated Care Board. The ideal candidate will have excellent communication and assessment skills. They will be able to work proactively to problem solve and prioritise their workload. About us ROLE PURPOSE / SUMMARY The role of the Mid and South Essex Integrated Care Board (MSE ICB) is to join up health and care services, improve peoples health and wellbeing and reduce health inequalities for our 1.2m population through our wider system and four Alliances. MSE ICB are committed to place clinical and professional leadership and the voice of our residents at the heart of realising all its ambitions. This role sits within the Nursing and Quality Directorate and supports the clinical and operational lead role in the All Age Continuing Care (AACC) service, which incorporates the adult Continuing Healthcare (CHC) and Children, Young Peoples Continuing Care (CYPCC) teams and the Neuro Navigation team. The AACC service delivers the statutory duties of the MSE ICB to assess and provide services that meet the identified needs of those found eligible for NHS Continuing Healthcare funding. The service delivers that duty through the implementation of the National Framework for Continuing Healthcare and NHS Funded Nursing Care (2018) and the National Framework for Children and Young Peoples Continuing Care (2016), working in accordance with the MSE ICB AACC Operational Policy. This Clinical Support Officer role will deliver a high-quality standard of clinical assessment, review and management of AACC cases, working collaboratively with key partners to achieve best outcomes. The post holder will work with the Clinical Support Manager to ensure that they deliver the highest quality and most appropriate services to patients and drive continuous service improvement. COMPETENCY BASED KEY DELIVERABLES Dependent on allocated caseload, the role may include: To provide advice and guidance to support both health and social care staff who are involved with patient assessment, care provision and care and support planning. To provide assessment, care and support planning, ongoing case management and review for all patients eligible for NHS continuing care funding. To assess retrospective claims for CHC eligibility. To support panels, appeals and local resolution processes as clinical representative. To ensure the agreed care and support package continues to meet the individuals assessed health and associated care/support needs and their agreed outcomes. Management of a small number of staff Contribute to the delivery of one or more strategic plans, enabling key milestones to be delivered and raising any issues associated with the delivery of plans and workstreams.
