An exciting opportunity has arisen for a dynamic and innovative senior Occupational Therapist within a specialist Dementia role. The post spans working with Cardiff Community Resource Team (CCRT) and the Specialist Memory Team (SMT), University Hospital Llandough. The focus of the role is to independently lead, develop, plan, organise and evaluate the delivery of Occupational Therapy services to patients who have a dementia and complex needs, and to offer support to their families. The role involves working closely with MDTs in both CCRT and SMT and developing relationships, links and learning opportunities across the teams. The post holder will assess, plan, implement and evaluate interventions for adults who live in residential nursing homes, private nursing homes, own homes and supported living accommodation The post holder will contribute to the planning, development and evaluation of Dementia Occupational Therapy Services within Cardiff & Vale University Health Board. This post is community based and involves travelling between patient homes in the locality, as such, the post-holder will be required to be a driver with a valid UK driving license, or the ability to travel between sites in a timely manner. Cardiff Community Resource Team is based over two sites in Whitchurch and Llanrumney. There is also a requirement to work from University Hospital Llandough and other UHB sites. The main duties of the role include . . . Autonomously managing a caseload of clients with complex needs, using evidence based / client centred principles to assess, plan, implement and evaluate interventions in primary care settings to include home visits and community facilities Providing leadership and education / training within the specialist field to clinical teams. Participating in the planning, development and evaluation of Occupational Therapy services within the role, holding responsibility for defined projects. Leading on the promotion and implementation of Local Government, UHB and RCOT strategies within the Occupational Therapy Team. Contributing to the maintenance and development of the Occupational Therapy services within the specialist Dementia role. Demonstrating a commitment to the education of Occupational Therapy students ensuring the maintenance of clinical standards through regular supervision of students on practice placement. Working in collaboration with the multi-disciplinary team and with primary care, voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface Maintaining clinical records in accordance with legal requirements and professional standard.
