Colne Valley Primary Care Network has an opportunity for a care co-ordinator to join our team on a full-time basis for 37.5 hours per week, over 5 days. You will provide extra time, capacity and expertise to our practice team to help improve the care provided to our patients. If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you. Provide patients with high quality, easy to understand information. Liaise with patients by letter, telephone, or text to ensure the relevant information and support is given to enable patients to attend all relevant appointments related to their clinical needs. Listen to a patients needs and signpost to the relevant services. Work with patients to provide appropriate information and ensure that expectations are managed. Contact and organise clinical reviews for patients where appropriate. Record an accurate and concise consultation within the patients notes using SystmOne clinical system. Liaise with appropriate clinical professionals to maximise patient needs. Build effective relationships with each PCN member practice and their staff. Work collaboratively with the current clinical teams through peer support and as part of the wider team. Build effective relationships with local system partners. Contribute to the evaluation of the service, collate or input timely data and suggest or implement any service improvements. "}
