Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system. Following the release of ARRS GP funding, we are looking for a newly qualified GP, to join Cygnet PCN on a fixed term salaried contact until 31 March 2024 initially, but this is likely to be extended. We will consider applications for between 4 and 8 sessions with a start date as soon as possible. Closing date is the 10th November 2024 . We are keen to interview candidates as soon as possible. You will be required to work within member practices (5 sites) supporting our surgeries through the winter period and beyond. All of our practices are rated good with CQC. We are a highly supportive PCN, understanding the importance of a good work/life balance with an open-door ethos. The GP will undertake a variety of duties governed by the GMS contract and objectives of Enhanced services, including: In Person & Telephone consultations, Patient Home visits and Nursing Home visits Conducting holistic long term condition reviews and managing the patients with evidence based best practice, Checking and signing repeat prescriptions and medication monitoring, Dealing with queries, paperwork and correspondence as required, Clinical administration including processing of results and coordinating referrals for specialist review, Working autonomously in providing routine pre-booked assessments, diagnosis, and treatment plans, Providing advice and support for the MDT members on site during your shift Opportunities to be involved, and at times, lead specific project work. To undertake duties governed by the PMS & GMS contract and Objectives of Direct and Local Enhanced A valid UK driving licence plus the use of own transport for business purposes is also essential. About us Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Screen patients for disease risk factors and early signs of illness. Cover the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate. Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well wider guidance such as that from NICE.
