The Community Response Team operates across the hours of 08:30- 21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated pro rata, we offer a mix of early, late and long days. To be successful with this post, you must live in the UK and have a full NMC registration. You must also possess a valid driving license and have access to your own car for this role. As a Community Nurse working in CRT you will be responsible for assessing, identifying and managing patients needs who have had a deterioration in their health. You will have a varied caseload seeing patients with long term conditions, falls, acute illness, supporting patients at the end of their life, and providing holistic, comprehensive care to patients in their own homes. The Community Response Teams' aim is to prevent hospital admissions and facilitate hospital discharges within a timely manner. You will also work closely with the Multidisciplinary Team, including: Therapists, GPs, Social Services, Urgent Response and many more. To undertake the first contact assessment, including planning, implementation, evaluation and reassessments of the care plans. In the absence of the District Nurse, to take responsibility for the day to day management of the nursing workload, delegating the workload appropriately to other team members.
