We would like an enthusiastic and highly motivated band 5 Community Nurse who will be happy to work in an evolving district nursing service. The successful applicant will be committed to working closely with health and social care colleagues to provide high quality care to patients with a wide range of medical conditions in the community setting. The role requires excellent communication skills as you will be undertaking complex nursing assessments and developing and delivering programmes of care for patients and carers. The district nurses work a 24hr service and there is an expectation that you may be required to support this to ensure service delivery. You will need to work collaboratively with other professionals, voluntary sector and informal carers to promote excellent care to patients in their own homes and community setting. You will be joining an enthusiastic and motivated team who are friendly and approachable. We are very patient focused and staff development is high on our agenda. As part of our team we want to develop all staff to the highest standard. You will need a full UK drivers licence and access to a car in order to fulfil this role. "}
