THIS POST IS FIXED TERM FOR 12 MONTHS DUE TO MATERNITY LEAVE. We currently have a vacancy for a temporary Band 5 Community Nurse post working 20 hours per week in Coedpoeth / Brynteg District Nursing Team. This post is temporary for 12 months to cover Maternity Leave starting from 24th November 2024. Both community home visits and Clinic based work will be required for this post, therefore knowledge of Leg Ulcer Management is essential. You will be based at Coedpoeth but the team work across both District Nursing Team bases. Coedpoeth/Brynteg DN Team cover 8 GP Practices and are an integrated DN Team. The post holder will be responsible to Band 7 Team Leader. The successful applicants will need to have excellent communication skills with an ability to provide clinically effective care within the District Nurse Team. The post holder is responsible for the assessment, planning, implementation and evaluation of evidenced-based nursing care, working collaboratively and co-operatively with others to meet the needs of patients and their families. Assist in the management and organisation of their clinical area and participating in the education, development and supervision of other staff members. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. "}
