You will also lead on audits and quality improvement initiatives, using data to inform service improvements and implement innovative solutions that enhance patient safety and experience. Collaboration will be at the heart of your role,you will work closely across our four District Nursing teams, as well as with specialist teams including the Bladder, Bowel and Pelvic Health Team and Urology to create seamless pathways of care for patients with complex needs. Expert Catheter Care: deliver high-quality catheter care to patients in their own homes, providing expert assessment, management, and support to ensure comfort, prevent complications, and promote independence Education & Training: Lead on catheter care training for district nurses, healthcare assistants, and other professionals, ensuring they have the skills and confidence to deliver safe and effective care. Audits & Quality Monitoring: Conduct regular audits on catheter management practices, identifying trends, monitoring compliance, and using data to drive improvements in patient care. Quality Improvement & Research: Develop and implement service improvements based on best practice, research findings, and patient feedback to enhance catheter care across the community. Collaboration with Specialist Teams: Work closely across our District Nursing teams, as well as with specialist teams including Bladder, Bowel and Pelvic Health and Urology to provide holistic, patient-centred care and ensure seamless pathways for those with complex catheter needs. Manage and implement change relating to the Bladder and Bowel service, monitoring and evaluating the change to improve quality of service provision. Develop and disseminate, in collaboration with key stakeholders, policies, guidelines and care pathways for Bladder and Bowel management, to promote evidence-based practice and integrated services. Maintain accurate records, submitting statistical returns and compiling reports as required. Practice in accordance with the NMC Professional Conduct and Practice. Deliver and participate in research-based practice to develop and maintain a high professional standard of care and expertise. Participate in research projects related to care and management of Bladder and Bowel conditions. Lead in the development, initiation and monitoring of ongoing audit programmes against guidelines, to identify gaps in service delivery and guide practice development. Manage and deliver nurse led community care for patients in their own homes for a caseload of people with Bladder and Bowel conditions Provide ongoing follow up care and support (home visits/ telephone support/nurse led clinics /outreach clinics) for people with Bladder and Bowel conditions.
