We are excited to be advertising for a Band 7 Specialist Nurse/Clinical Team Manager to lead Early Intervention for the Specialist Learning Disability Service. The Specialist Learning Disability Service is a Specialist service consisting of Learning Disability Nurses. Clinical Psychologists, Occupational Therapists & Assistant Practitioners who offer a range of services for children and adolescents with a diagnosed Learning Disability and Autism from assessment and intervention to consultation, liaison and training for other agencies. The service can support Children & Adolescents with a moderate to profound Learning Disability, in conjunction with complex behavioural, health or Sleep issues related to this diagnosis. We provide training in Positive Behaviour Support, Specialist Sleep Training, FAIT (Frankish Assessment on the Impact of Trauma) and Talking Mats . Bespoke training is also considered if required for the role. Work within the Specialist Learning Disability Service providing support and supervision to Learning Disability nurses working within the Health Pathway and Learning Pathway and all other integrated pathways. To develop the existing Health Pathway to provide specialist support for CYP with a Learning Disability and support processes for reasonable adjustments for appointments and hospital admissions. To work alongside the other Pathway Lead's in developing a service that is responsive to the need of CYP with a Learning Disability. Screen and triage referrals and ensure a strengths-based approach to assess, plan, implement and evaluate interventions to support complex packages of care for children with additional needs in the community Contribute to risk assessment and care planning in collaboration with children, young people and their carers "}
