The post holder will be required to work across both our North locality teams covering Huntingdon and Peterborough. The base for the post can be either Kingfisher House, Huntingdon or Phorpres Close, Peterborough. This position is working with people in their own homes and private provider care homes supporting patients out of hospital and prevention of admission. Applicants will be offering holistic clinical assessment, treatment, discharge planning and specialist therapy knowledge. Successful applicants will play a vital role in supporting patient flow through the pathways as well as providing clinical skills and knowledge to the MDT.
