This service delivers specialist respiratory care to patients in the community to improve the self-management of their condition. We provide domiciliary visits and clinics in venues such as Health Centre's and Community Hospitals. The team is a mixture of health professionals including Nurses, Physiotherapists and Occupational Therapists working across the interface of primary and secondary care. You will be providing a mixture of general and respiratory care to a group of patients, be proficient and competent to work at a high competency level, and have a NVQ level 3 in Health and Social Care or a Certificate in Spirometry. You will be expected to support the delivery of Pulmonary Rehabilitation as part of the role, and as such enthusiasm and experience within Group Physical Activity would be beneficial. You will also be expected to perform standard clinical assessments, within your level of competency and supported by registered healthcare professionals, keeping accurate records for review as well as perform domiciliary visits at the request of senior colleagues. The ability to perform general office duties to support the team is also required and therefore an excellent telephone manner and experience of using Microsoft Word, Excel, email and databases are essential for this post.
