The post holder will be employed by Crane Valley and will work for Verwood Surgery. We are looking for a collaborative and innovative ANP to work closely with clinical teams to deliver patient care. The post-holder will: Work independently to offer direct access for the general practice population via triage clinics, conducting appointments either in person or via telephone. Serve as the initial point of contact within the Practice for patients presenting with undiagnosed or undifferentiated health issues, utilising skills in history taking, physical examination, problem-solving, and clinical decision-making to establish a diagnosis and develop a management plan, including home visits if needed. Participate in a rota to support the triage of daily patient requests, making immediate decisions and redirecting patients to the most suitable clinician within the appropriate timeframe. May include working some extended hours. Utilize advanced professional accountability, autonomy, and judgment when assessing and treating patients with diverse situations and conditions. Assist in achieving management team objectives where possible. Participate in mandatory and internal/external training as required. Adhere to all policies and protocols in the Staff Handbook and contract, demonstrating behaviours aligned with Verwood Surgery values. About us The Advanced Nurse Practitioner(ANP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients. The ANP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment. ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis, and interpretation of test results. They can independently prescribe appropriate medication, evaluate, or refer to other specialists if necessary. Primary Key Responsibilities The following are the core responsibilities of the Advanced Nurse Practitioner.
