On a Wednesday we also provide extended hours till 8.30pm (rota basis) and on these days you would do an afternoon session instead of a morning. We are currently 3 partners and 7 salaried doctors supported by 2 ANP's and a strong nursing team serving 12,600 patients in Wilmslow. We are total triage practice with a great reputation within the community and score highly with friends and family surveys. We would welcome you to our stable clinical team practising from modern and refurbished premises. We are looking for a committed, motivated and innovative hardworking GP to support us in providing our patients with the highest standards of care. The practice is supportive of personal development. Informal visits and enquires are welcome. Interviews to be held Tuesday 12th November PM and Wednesday 13th November PM. In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness. In consultation with patients and in line with current Practice disease management protocols, developing care plans for health. Be responsible for a cohort of patients involving care planning and repeat prescribing. Providing counselling and health education. Referring to other care providers as appropriate. Recording clear and contemporaneous consultation notes to agreed standards. Collecting data for audit purposes, for the Quality & Outcomes Framework and for Enhanced Services. Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible). Prescribing in accordance with the Practice prescribing formulary (or generically) and in accordance with the practice prescribing plan whenever this is clinically appropriate Work in line with practice policies in support of PCN mandated working, making efficient use of the ARRS roles staff and to support the development of the PCN. Be responsible for a designated clinical domain to lead on within the QOF and produce updates at practice meetings as required and be responsible for the oversight of this area.
