Please see job description for further details We are looking for an enthusiastic individual to fill our Band 3 Community Health Care Support Worker role within the Urgent Community Response team covering Mid Hampshire. The support worker role in the Urgent Community Response Team is an exciting opportunity to support people to remain at home, when they are at risk of being admitted to hospital through recognising the deteriorating patient and escalating appropriately alongside delivering delegated interventions. The team is led and supported by both advanced and consultant practitioners, alongside the team lead. We will support you to develop the skills needed to effectively support and rehabilitate people following a patient centred care plan. You are able to work in a holistic way, while having the ability to develop your skills. The role will involve supporting the team in the ongoing care and interventions for people with acute health needs alongside their long-term conditions. This can vary from providing acute rehabilitation to physiological monitoring and recognising signs of deterioration. The central feature is putting the patient first and enabling them to remain safely in their chosen place of care. Maintain excellent communication with patients relatives and members of the team regarding aspects of care demonstrating a variety of communication skills in accordance to patient need. Communicate on complex and sensitive issues. Maintain clear concise and legible documentation in accordance to policy. To support the work of the registered practitioners in the team. To prepare patients for and assist with clinical therapeutic activities or treatments or investigations and procedures. Perform patient assessment under supervision and after appropriate delegation from the registered practitioner plan and deliver high standards of care. Perform area specific clinical competencies to enhance the delivery of patient care. Provide concise handovers to other members of the team. Documentation must be clear concise legible and signed. Utilise organisational risk assessment tools appropriately and act on the findings. Take the appropriate action in response to the patients vital signs after reporting to a registered practitioner. Identify and report to a registered practitioner clinical risk issues that impact on the patient clients. Provide and promote health education specific to the clinical area and in line with national and local policies. Assist and support patients clients in the activities of daily living. Practice corporate values and contribute to team and departmental objectives. Attend staff meetings and contribute to the coordination and development of services as required. "}
