Assertive Outreach West have a great opportunity become available for a full-time permanent position. We are seeking a registered Mental Health Practitioner to fulfil this role. The post is an exciting opportunity to broaden skills within the Community expanding current knowledge and competence working with clients who have a severe and enduring mental illness. We as a team encourage social inclusion and working with the client to live the best life that they can. The role is not exclusively for Band 6 and we would be willing to consider a band 5 position for the right candidate. We look forward to a motivated candidate joining our well-established team in meeting the needs of our service users. You will work as part of an established integrated multi-disciplinary team made up of Consultant Psychiatrists, Psychologists, Nurse therapist Mental Health Nurses, Support Time Recovery Workers, Occupational Therapists. The Assertive outreach Weymouth team covers pan Dorset County, with the exception of Bournemouth, Poole and Christchurch. You will need to have a valid driving licence and use of a car as you will be required to assess and monitor patients in their own homes. Assertive Outreach is a Monday to Friday service. This post is for 37.5hrs per week, working Monday to Friday, 09:00hrs to 17:00hrs The primary role responsibility is being a dedicated Care Coordinator for a caseload of patients with whom you'll provide person centred targeted support to assist in aiding their holistic recovery. There are opportunities for you to advance you own clinical assessment skills whilst risk assessment will remain at the forefront of everything you do. Alongside this there will be completion of regular clinics i.e. depot, Olanzapine and physical health. You should have recent experience of working in a mental health environment and will need to be able to apply the philosophies of engagement and recovery in your work with service users. It goes without saying that you are reliable, committed and motivated with excellent communication, organisational skills with the ability to prioritise. In return we are ready to offer you comprehensive structured management supervision, management you can approach any time and peer support which is second to none. In addition to this we are also happy to support you in advancing your own skill set using additional learning opportunities such and NMP, ACP and psychological therapies. The post is for 37.5 hrs covering Monday to Friday, 09:00hrs to 17:00hrs. Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.
