You will work with residents and care home staff to assess resident's needs, putting a plan of care place, and deliver training with the purpose of reducingavoidable ambulance call outs and hospital admissions. We are looking for a self-motivated, autonomous, proactive registered nurse committed to their professional development, bringing their skills and knowledge to the team. You will be based in the community working in collaboration with a network ofhealth and social care professionals.
