They will record clinical activity accurately and comprehensively and submit this promptly to the Information Department.They will also be expected to participate in service and business planning activity for the locality and, as appropriate, for the whole mental health service, including participating in annual appraisals. Clinical Duties of the post holder The post holder will be expected to give support to the consultant and the ADAPT team in developing and maintaining the caseloads of all patients ensuring that Core Assessments, Risk Assessments and Care Plans are available in RiO case records. To provide reviews including Mental Health Act where necessary to the ADAPT team and to develop comprehensive case-note summaries, comprehensive multi-axial assessment processes and care planning in line with NICE guidelines. The post holder will also be expected to attend ADAPT team meetings and toensure the involvement of families and carers, wherever possible, with the patient's consent in treatment planning. To always support the consultant in preparing any necessary reports, and attending Mental Health Review Tribunals if applicable. They will also provideclinical cover for the consultant, when she/he is away on leave, with supervision by another consultant. "}
