Attend education events regarding gastrointestinal cancers/FIT pathways This post is to support and facilitate patients who have a positive FIT test and or are referred on a 2 week wait fast track referral for a suspicion of a gastrointestinal cancer. Working with the MDT Clinical and Admin staff to ensure patients are treated within a timely manner and to improve the 28 Faster Diagnosis Standard. We are looking for a highly motivated individual who has NHS experience to facilitate and navigate this complex cohort of patients pathways. The need to work autonomously with minimum support following intensive guidance and induction is paramount within this role. Monitor all patients with a positive FIT test result and navigate all patients referred on a gastrointestinal suspicion of cancer pathway from point of referral to support them to attend all appointments and diagnostics within their pathway. Instruct and monitor patients on undergoing FIT diagnostics and chase results. Support service improvement and undertake audits with support to track processes and improve outcomes Cover LGI/UGI MDT Pathway Coordinator to facilitate and manage MDT weekly meetings. Liaise with patients/families/clinical and admin staff within Primary Care and secondary Care to ensure patients are treated within a timely manner in line with the Bradford and Airedale FIT Pathway Guidance.
