About us Job Summary Undertake individual Children's Continuing Care Assessments in line with the National Framework (2016) Case Management. Undertake verification of completed Assessments undertaken by the team. Support, encourage and develop the skills of the wider team. Work with providers of health care in the home / school as required. Chair meetings as required requested. Present assessments at a wide range of meetings and panels. Support CCC eligible individuals and or their representatives. Support the development and review of CCC PHBs in line with the CCC National Framework (2016). Inform the targeting of resources, monitoring, implementation and evaluation of Packages of Care and PHBs by providing high quality support including complex information and analysis, communications and stakeholder management both within the wider CCC Team and beyond. Ensure accurate and open communication and co-ordination with a range of organisations and individuals, researching and drafting correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialties, functions and projects including contributing to policies, procedures, pathways and CCC local and National frameworks. Support in the strategic development and oversight of local transition planning processes and planning meetings. Undertaking CCC assessments and care planning to ensure smooth transition to adult services.
