Are you looking towards a change of direction in the development of your career? Building on our current success we are developing our Childrens Continuing Care team. An exciting opportunity has arisen to recruit to this new post, and we are looking for a motivated and enthusiastic paediatric nurse to join us. The successful candidate will have specialist paediatric clinical knowledge and experience, will be an effective communicator, have an ability to develop effective assessment skills; will be an effective leader and manager, will be resilient and able to work autonomously but also work as a valued member of a team. Good knowledge of safeguarding and multi- agency working is required. This exciting new post is a community-based post with the post holder expected to travel throughout Gloucestershire. Occasional out of County travel may also be required. Key Working Relationships Required to maintain constructive relationships with a broad range of internal and external stakeholders. Participate in relevant internal and external working groups/projects, services, and initiatives which may be highly complex, sensitive, political, and contain contentious information with the aim of providing information and analytical advice to the teams. Work with members of the team to develop and implement project data collection systems that will provide accurate and timely data Communicate information, risks, issues and dependencies, including briefings and reports to Project teams, sponsors and a range of internal and external staff. About us Job Summary Undertake individual Children's Continuing Care Assessments in line with the National Framework (2016) Case Management. Undertake verification of completed Assessments undertaken by the team. Support, encourage and develop the skills of the wider team. Work with providers of health care in the home / school as required. Chair meetings as required requested. Present assessments at a wide range of meetings and panels. Support CCC eligible individuals and or their representatives. Support the development and review of CCC PHBs in line with the CCC National Framework (2016). Inform the targeting of resources, monitoring, implementation and evaluation of Packages of Care and PHBs by providing high quality support including complex information and analysis, communications and stakeholder management both within the wider CCC Team and beyond. Ensure accurate and open communication and co-ordination with a range of organisations and individuals, researching and drafting correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialties, functions and projects including contributing to policies, procedures, pathways and CCC local and National frameworks. Support in the strategic development and oversight of local transition planning processes and planning meetings. Undertaking CCC assessments and care planning to ensure smooth transition to adult services.
