The facility is an important phase in the patient pathway to facilitate early release of inpatient beds for new admissions and provides a comfortable and calm environment for patients to complete final treatments and discharge arrangements. The facility will provide a nurse led service accepting patients on the day of transfer/discharge ensuring a smooth, timely and safe transition from hospital to home or other specified destination. We are therefore seeking a driven, enthusiastic and focused leader who will lead the nursing team thus enhancing the patient experience, improve patient outcomes and contribute to organisational priorities of improved patient flow.
