The Early Intervention Team (EIT) is a proactive and innovative team of Discharge Planning and Frailty Specialist Nurses, Occupational Therapists, Physiotherapists and Social worker. EIT provides a proactive Admission Avoidance service, Early Discharges, Complex Discharge Planning, Frailty screening, Therapy assessments for patients admitted to the Emergency Department, Assessment Units (SDEC), Frailty Unit, Short Stay Wards and in-patient areas. The service provides an excellent opportunity to develop quality improvement and service development skills as part of the service expansion. About the role We are looking for an innovative and motivated team leader. You will support frail older patients. You will provide a specialised assessment and offer recommendations in order to support their admission in CUH. You would be joining an established senior tier and would be responsible for the training and supervision of the junior team members. Development opportunities will include continuing professional development, links with community services, representation to key operational group meetings, and striving to meet our national and local best practice guidelines. The core hours are 8am to 8pm seven days a week based on a rota system. To work with the service Lead and other Band 7 clinical lead Nurses and Therapists to develop the service to best meet changing health and social care agendas. To ensure the capture of data and reporting of Key Performance Indicators for the service. To identify and lead on quality improvement projects evaluating the service against the service objectives.
