To provide leadership for patient safety throughout the organisation and ensuring that safety is prioritised and implemented in line with national guidance and the Trusts contractual requirements. To provide assurance to the Trust Board, our commissioners and regulators ensuring a high-quality approach to patient safety is embedded. To lead the Trusts Mortality & Morbidity process ensuring compliance with national guidance and requirements. To support the implementation and embedding of the NHFT patient safety strategy The post holder will: Provide line management to the patient safety team, including specialty leads such as Falls Lead and Medical Devices Lead Provide expert knowledge to all members of staff on patient safety which includes, but not limited to, incident management, learning from incidents, human factors analysis, national patient safety guidance and initiatives and learning from deaths Through management of the Mortality Lead, ensure the Trust has processes in place to respond to and learn from unexpected deaths effectively and efficiently. Quality assures the processes ensuring the Trust is meeting national standards and targets. Ensure the Learning Disability Services are utilising and adhering to the LeDeR Programme through support and education Support the Trust to implement and deliver the requirements set out in NHS England's Patient Safety Incident Response Framework (PSIRF) and collaborate with relevant services to facilitate and embed learning from patient safety incidents. Support annual review of the Trust's Patient Safety Incident Response Plan (PSIRP), in line with the requirements of PSIRF, provide detailed analysis and triangulation of patient safety data from a range of IT systems (including, Datix, SMART reports, Learning From Patient Safety Events (LFPSE), StEIS) for ongoing quality improvement projects that directly affect patient outcomes.
