The post holder will be responsible for the planning and delivery of patient care within Adult Community Nursing Teams. To assist in supporting the coordination of community nursing services across Wakefield District, to ensure uninterrupted, seamless patient care, to support nursing colleagues in the provision of a high quality, 24 hour, 7 day per week nursing service.
