Ensure the Head of Department, Lead Nurse and the Commissioning team are kept fully aware of any cause for concern in any care environment; liaising as required with the CCG Quality Team. The post holder will be responsible for the assessment of the individuals in the community/ home/ nursing homes and hospitals to determine eligibility to receive NHS Continuing Healthcare or Funding Nursing Care. The assessor will act as a member of the multidisciplinary team to co-ordinate the process, establish eligibility and assist in identifying a suitable care package. The assessor will be responsible to ensure the review process for all NHS Funded Care is undertaken in accordance with current guidance. The practitioner will utilise the national assessment tools and adhere to the required timescales, co-ordinate, participate and support the assessment process with the multidisciplinary team.
