Ensure appropriate records are maintained under the direction of the qualified practitioner. Contribute to the involvement of service users, and carers/relatives in the delivery of care Provide reassurance and support to service users and carers/family Work collaboratively within the Care team to ensure that service users needs are met Recognise and respond appropriately to urgent and emergency situations when directed by qualified practitioner Contribute to the development of services Contribute to the effective and efficient use of resources Understand and adhere to Trust policies, procedures and clinical guidelines Report any concern regarding service user care to the Qualified Practitioner Contribute to service improvement initiatives to enhance quality of service user care Participate in service user and public involvement activities Support individuals equality, diversity and rights Participate in Serious Untoward Incident investigations/reviews as required An exciting opportunity has arisen within the Warrington Community Learning Disability Team for a Community Learning Disability Support Worker to support the delivery of specialist health support which promotes the independence, community inclusion and positive reputation of people with learning disabilities. The post holder will assist in the delivery of care plans and programmes by - implementing programmes and reporting outcomes, contributing to the recording of plans/programmes and providing therapeutic activity where agreed and appropriate. Acting to as a nominated worker for individual clients who have learning disabilities and may also have extreme challenging behaviour including physical and verbal aggression, changes in mental health, forensic needs, complex physical health needs. Facilitate and support the development of independence with activities of daily living. Support self-help, communication and social skills acquisition The service model is one of supporting people, whose behaviours challenge services, working closely with specialist practitioners and other health professionals to support this complex group of individuals to remain in their community setting and, where an appropriate admission has occurred, to be discharged promptly and safely in line with national Transforming Care strategy. To promote and champion the rights of people with learning disabilities, working with the current service philosophy and values as detailed in Transforming Care for people with a learning disability Support the delivery of specialist health support which promotes the independence, community inclusion and positive reputation of people with learning disabilities.
