We are seeking a Specialist Practitioner to work within our team to provide assessment, diagnosis, and pharmacological management of patients. This is a home-based, virtual role and you will be provided with all the kit and training needed. What we need: Passion, enthusiasm, and dedication Commitment to excellence in delivering patient-centred care Demonstrable experience within a similar role Good understanding of NICE guideline (NG87) and quality standards (QS39) for ADHD in adults Confidence in working autonomously Current registration with the NMC, HCPC or other appropriate professional body Independent non-medical prescriber status About us PHL provide a service to the Isle of Wight, receiving referrals for adults presenting with symptoms suggestive of Attention Deficit Hyperactivity Disorder. Patients engage with triage, assessment and, if receiving a diagnosis, may subsequently choose to proceed to pharmacological management of their condition. This is a virtual role for a non-medical prescriber with experience of assessing and treating adults with ADHD. You will be working alongside the Clinical Manager and other professionals to diagnose and manage patients within a virtual clinic setting, including prescribing and titration of preparations in line with NICE guidance and clinical judgment. You will have a sound understanding of the relevant NICE guidance (NG87) and quality standards (QS39) for ADHD in adults and have experience of using DIVA to diagnose ADHD. You must be confident in considering other psychiatric co-morbidities when undertaking assessment, ensuring that the presenting symptoms are attributable to ADHD. You should also have sound understanding of the therapeutic efficacy, side effects and interactions of any prescribed treatment. You will work within a small team to provide excellent patient-centred care, participating in multidisciplinary meetings as required to review individual cases. As this is a virtual role, you must be confident in conducting assessments by either phone or video consultation. PHL will provide full training and support to use the applications and equipment required. Clinical Work autonomously within the wider team, demonstrating accountability, responsibility and professionalism when providing a quality service to our patients. Undertake assessments for the purpose of ascertaining whether a patient meets the criteria for a diagnosis of ADHD, using both clinical acumen and the DIVA tool. Formulate a management plan for our patients, including treatment pathway, follow-up and review, discharge, and onward referral, as necessary. Actively promote self-care and health strategies to empower the patient, taking a holistic and person-centred approach. Identify and appropriately refer to the patients own GP for vital signs or ECG monitoring in line with NICE guidelines. Identify when patients require the input of external agencies and refer to primary, community, non-profit and secondary care services. Identify child and adult protection issues, social needs, and safeguarding concerns and action these in line with protocols. Record in a timely manner a true and accurate record of the information and advice given, and to generate an electronic record. Undertake assessments of risk at each consultation, implementing onward referral and risk management plans. Be responsible for timeliness of consultations, within the booked appointment system. Always ensure the safety of patients, including provision of safety netting advice. Provide cover in the absence of the Clinical Manager, undertaking additional responsibilities as agreed, once confident and competent to do so. Prescribing Prescribe appropriately for patients requiring initial or repeat prescriptions, in line with NICE guidance and the BNF. Conduct medication reviews as required, titrating dose and/or prescribing alternative treatment as dictated by patient need. Work independently in reviewing medication, understanding boundaries of knowledge and when to refer patients for further management with a relevant service.
