Our vision is to develop a multi-disciplinary community urgent response service to support some of the most vulnerable patients, providing responsive capacity to our GP Practices and local health and care system. You will be part of a team that supports the Ambulance Service to refer patients who, with support, are able to remain at home. This service will reduced conveyances and enabled paramedics to reduce the time spent in a patient home. This will be non planned, reactive work to see patients who have been in contact with the Ambulance Service. This service is 8-8 working on a 7 day service. Various shifts available. To work as an autonomous practitioner Attending to patients who wish to be seen by a health care professional / phone consultations / triage. Diagnosing, treating, and prescribing for patients presenting with undifferentiated and undiagnosed illness or injury. Assessing patients needs, based on highly developed skills sometimes including physical assessment. Overseeing and advising people in respect of their continuing medical needs. Patients are to be seen in a timely manner enabling contractual requirements to be met. The Primary Care Paramedic will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. There will be a requirement to see extra or emergency patients. As a clinician be available to support other healthcare team members and to accept referrals from other team members. To liaise with all members of the Primary Health Care Team and other agencies local authority, social services, secondary care, voluntary sector, and Primary Care in order to assure appropriate care is provided for the practice population. To participate as a key member of the multi-professional team through the development of collaborative and innovative practice You will be trusted to manage your time effectively, ensuring HCAs receive support and training in a timely way.
