This post will enable you to develop your skills in caring for patients towards the end of life and supporting them to be cared for in the place of their choice. The postholder will work flexibly on the Night Response Rota, Taking calls coming into the Hospice overnight, providing generic advice, signposting and redirecting calls to other services available as appropriate. Acting as a resource and mutual support for other members of the team. You will liaise with other staff on the In Patient Unit to sustain and develop the relationship between The Princess Alice Hospice Services and the patients/families receiving palliative care support by the Hospice. Experience in caring for patients who are terminally ill is essential. Applicants will need to have their own car, and a full driving license. As you will be visiting patients in the community, you'll need to be a car driver with full driving license. About us When working a shift on the Community Nursing Service, responsibilities will be carried out under the guidance of the patients Clinical Nurse and ESS Lead. Carries out all personal care and activities of daily living for individual patients and groups of patients as required. Carries out delegated nursing activities to assist nursing staff, e.g. catheter and stoma care, eye and mouth care and report findings to nursing staff. Keeps nursing staff informed of observations and changes in condition relating to patient care. Assists with general orderliness and homely atmosphere of the patients own home as appropriate Develops and maintains effective communication and liaison with patients, their families and carers. Becomes familiar with, and follows the agreed Hospice policies and procedures Contributes to maintaining the Hospices agreed philosophy, aims and objectives. Works as part of the team to promote an atmosphere in the Community conducive to lifelong learning and high standards of care. Promotes a safe environment for all patients, carers, visitors and staff in the Ward or the patients own home as appropriate. The post holder may be asked to support the Hospice In-patient Unit on occasions where capacity threatens the fundamentals of core delivery. In these instances responsibilities will be carried out under the guidance of the Ward Manager / Clinical Lead and the Team Leader for the shift. "}
