To evaluate patient progress, reflect, analyse and reassess intervention and alter treatment programmes, as appropriate. Clearly communicating any changes in care plan or treatment approach to other team members to ensure consistency of approach and enhance recovery. You will work as part of multi disciplinary team, alongside physiotherapists, nurses, support workers, associate practitioners and consultants. You will be responsible for the assessment of patients allocated to you, and for the provision of their treatment plan and subsequent interventions, You will be required to complete joint visit with other members of the team. To be able to feedback to the coordinators in the service with a clear plan for each patient. You will need to complete on ward referrals to other services, and ensure and make recommendations for the provision of equipment and or adaptations if deemed appropriate. You will also be involved in the assessment of patients with cognitive impairment and dementia and will be expected following appropriate training to be able to take patients observations, and complete clear concise and appropriate records.
