The successful individual will be based at Ludlow Community Hospital. They will join an enthusiastic and supportive multi-disciplinary team in providing high quality care and rehabilitation. The successful individual must have an interest and experience in inpatient rehabilitation and be able to work as an autonomous practitioner. They will be working alongside colleagues to contribute to safe and effective discharge plans, taking an active part in Red2Green approach to help reduce unnecessary delays for patients. This post will be involved in supervision of junior staff, rehabilitation technicians and students. To provide a key role in facilitating the seamless transition from community hospital to patient's own home and transferring the care of the patient to other appropriate services to maximise the patient's independence and quality of life.
