To record clinical activity accurately and comprehensively, and submit this promptly through the trusts clinical information systems.5. About us Principle Clinical duties of the Kilburn post holder: Assessment of new patients and clinical supervision of assessments carried out by other team members. Full participation in the multidisciplinary team providing care in the Senior medical responsibility for the care carried out by the Community Mental Health Team. Review and monitoring of the caseload to ensure appropriate care packages are put in place and discharges facilitated to maintain patient flow. Contribution to the continuous development of the Community Mental Health Team, and to other parts of the Service as required. Domiciliary visits to provide assessment and treatment services as a member of the CMHT Domiciliary assessments in response to emergencies Research, in terms of personal projects and clinical audits as part of clinical Involvement in joint planning of future service Occasional psychiatric assessments of residents who may be in crisis in another Close liaison with the wards to facilitate admission and discharge Liaison with HTT, Psychiatric Liaison Services, AMHP services and Forensic Services in the borough. Principal Clinical duties of the EIS post holder To be responsible for carrying out detailed psychiatric assessments and risk assessments and to contribute to planning and implementing service users care plans. To carry out the above in a variety of settings, including home visits, and other non-stigmatising settings. To work flexibly to ensure that clients and their carers are able to access the service effectively; To review clients within the community as well as those who require a hospital admission for medical assessments and evaluation including physical and mental state examinations. To be an active member of the multi-disciplinary team. To take turns in chairing the daily MDT with the Team Leaders and maintain adherence to the Zoning protocol. To involve the user and carers in the treatment, management and care planning of a users mental health and physical care. To liaise effectively with appropriate agencies, statutory and non-statutory, other mental healthcare providers, GPs and families and carers, as appropriate, regarding clients.
