The team work closely with Adult Acute, PICU and Rehabilitation wards to help facilitate safe and effective admission and discharge to and from inpatient settings. The post holder will work under the supervision of a team leader and joint work with social work and inpatient colleagues. Main requirements are to: Contribute positively to multi-disciplinary and multi-agency team workSupport people in Adult Acute, PICU and Rehabilitation Inpatient services aged between 18 and 65 who have severe and enduring mental illness Work with individuals to develop support plans, set outcomes and enable people to achieve those outcomes. Support individuals through the recovery process, to achieve the best quality of life possible and to be able to live as independently as possible. Identify and support with overcoming barriers to discharge About us To liaise with the Registered Social Care Professionals and carry out assigned duties delivering a variety of mental health and social care interventions to service users suffering from mental ill health. Provide direct social care support including short term social work intervention, rapid reablement and longer-term social care intervention under the direction of a qualified social worker, where applicable for joint-worked cases. Provide direct physical, social and psychological care to service users under supervision in accordance with individualised support plans, reporting about the service users condition to qualified staff and enabling service users to achieve defined outcomes set out in their support plans. Exchange factual information with service users, who may have barriers to understanding using reassurance, tact and empathy. Contribute to the assessment and analysis of clinical and non-clinical situations, making routine judgments to resolve straight forward problems. To be prepared to work on own initiative as necessary and report to social care professionals. Promote equality for all service users and understand their social care needs. Demonstrate an awareness of service user and carer perspectives on the provision of social care. Comply with and carry out safe practice in accordance with Trust policies and procedures and Social Work England codes of conduct.
