Then this is the opportunity for you. We are based in an vibrant area of the North East which includes historical sites, beautiful coastlines and world class eateries. Gateshead CTT are looking to employ a suitably qualified Community Mental Health Practitioner. This is a full time posts (37.5 hours) for a Registered Nurse who must be registered with the NMC. Here within the Gateshead Community Treatment Team there is a real sense of team spirit, and we are looking to add clinicians with this same positive outlook to our already excellent skill set. The mental and physical wellbeing service is a nurse led clinic within the Gateshead Community Treatment Team, for the ongoing treatment and monitoring of service users with long-term mental health conditions. Service users within the clinic will have reached a level of stability within their recovery, but have ongoing chronic difficulties and risks which require ongoing treatment and monitoring. Appointments are offered on a one to basis within a clinic setting. The role would be to monitor and review medication including Clozapine, Lithium, high-dose antipsychotics therapy and ensuring care plans meet the associated physical health monitoring requirements. Risk assessment is vital and the ability to assess and refer on to other relevant pathways such as, enhanced care CPN case load, Crisis Team and hospital where indicated. The practitioner will be required to maintain excellent care records in line with care programme approach requirements, and maintain excellent communication with GP's. You will be able to develop your skills by working with service users who have a range of complex mental health needs with associated risk, and you will need to demonstrate experience of being able to plan and deliver in collaboration with service users, families and their carers.
