This is a 28 bedded unit focusing on rehabilitation of the elderly. The service is both GP and consultant led. Admission are accepted from the community and both general hospitals within Carmarthenshire. The team work along side community, primary care and the acute sites to develop an integrated rehabilitation approach to promoting patient independence and best outcomes for discharge. The candidate should be able to demonstrate strong communication and negotiation skill in order to use and apply this knowledge and skills to facilitate complex discharges. The candidate must hold level 1 registration in nursing have evidence of on going CPD and have knowledge of multi agency working, community services and awareness of the Continuing health Care Process. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Interviews to be held on 22/11/2024.
