Working at an advanced practice level, you will complete comprehensive, holistic assessments including developing individualised palliative and end of life care and treatment plans, using advanced care planning skills, in the community and outpatient settings across the island. You will manage your own caseload, ensuring that you provide totally holistic care to the patient and their family. Responsible for managing your own caseload of patients and for supervising junior members of the nursing and care at home teams as required, using recognised referral guidelines and agreed levels of nursing intervention. You will incorporate a multi-professional approach across all health, education and social care settings to enable the highest standards of care to be provided to your patients, their families and carers. About us The post holder will work autonomously and as part of a team contributing to the provision of 24/7 Specialist Palliative and End of Life Care, providing an environment that encourages patients and their families to play an active part in their care. The successful candidate will provide advanced levels of clinical decision-making through assessment and interpretation of clinical information to support patients, families and carers who have complex or rapidly changing physical symptoms, emotional or practical needs. There is a requirement to work days, nights and weekends on a rota basis - shift allowance given for weekend, evening and bank holiday working.
