The successful applicant will be responsible for tracking cancer suspicious/ cancer confirmed patients through their pathway, from the point of referral to treatment, in line with national standards. The applicant will be responsible for ensuring timely escalations and treatments. The role is required to work closely with the operational service ensuring that patients pathways are expedited as required. The role involves using multiple hospital systems, including PAS, ICE and Somerset. The post holder is expected to cover other team members during periods of absence and increased activity. This post is a 1 year fixed term position Previous applicants need not apply. Undertake patient-level tracking duties covering elective, cancer and specialist patient pathways, with primary responsibility for patients on a cancer pathway. Ensure the pathway information is recorded accurately on Trust systems including Digicare Expanse & Somerset systems. Provide support for the performance processes of waiting list management and performance reporting, which require review of data and patient level information. Escalating patients to relevant departments and managers where progress is not satisfactory in accordance with the clinical pathway. Support colleagues within the Trust in understanding pathway management, rules and principles; acting as an expert in pathway measurement. About us Review and interpret patient encounters and pathway using multiple clinical systems and patient records to determine the key dates and timelines for patient care. Tracks the progress of patients through key clinical pathways including cancer pathways and cores services such as radiology and pathology. Escalating patients where progress is not satisfactory in accordance with the Cancer Access Policy and standard operating procedures. Review data extracts and validate event dates against patient care information held electronically and within paper records to determine the correct information to be recorded on Patient Administration & Clinical Systems. Utilise Trust systems to collect and monitor information, maintains and updates relevant systems and databases as required. Provide waiting list and patient tracking list validation support to enable delivery of operational services including the escalation of patients who are still awaiting clinical care and are not progressing along their pathway in a timely manner. Provide support for administrative and clinical pathway management processes including providing patient-level information to PTL meeting and MDT meetings. Ensure data correction is undertaken to ensure accurate patient data is held on Patient Administration & Clinical systems for reporting and other purposes. Assist with the maintenance of accurate and complete audit logs of validation and data correction work carried out. Liaise with clinicians, nursing staff and other staff as appropriate in order to achieve collection of accurate data for validation and correction purposes. Support Trust and departmental processes for the review and management of data quality issues related to performance and patient pathway activity. "}
