Chippenham Community Team have an exciting vacancy for a Rehab Support Worker. The role of a Rehab Support worker is to work closely with a wider team to support individuals in their own homes for a short period after discharge from hospital. We provide support and rehabilitation to promote independence in activities of daily living. The role also includes supporting our therapy team with home exercise programmes and provision of equipment, following care plans and providing feedback. Our service is provided between 08.00 and 21.00 with shifts of 08.00 14.00 and 15.00 21.00. This post is for 37.5 or 30 hours per week over 5 days per week and will include alternate weekends. You may also be expected to support other local teams across North Wiltshire. The successful candidates will need to be able to provide clear evidence of having previously worked in a care role, preferably with older people or people with long term conditions. We would either like you to have a Care Certificate and NVQ Level 3 apprenticeship or willingness to study to that level. We have a range of training and development opportunities, including an induction program and ongoing education. Due to the strong emphasis on rehabilitation, training in rehabilitation focused care will also be provided. Applicants need to have enthusiasm and commitment to working within a community-based team and to have recent experience of working in or in partnership with community services within the NHS or Social Care.
