We currently have a vacancy for 2 days per week on a Monday and Tuesday to support our current projects above. Core Hours of Service: Monday and Tuesday 8.30am - 4.30pm Experience of performing structured medication reviews is preferable but not essential. We will provide training as part of an individualised induction programme and you will be embedded within an experienced frailty team. The successful Pharmacist will have a focus on patient centered care, and an interest in frailty, they will demonstrate excellent communication and organisational skills. Applicants are welcome from all pharmacy sectors. You will receive professional support from our established Medicines Service team based in Hull. You must have an independent means of transport to travel around the geographic area our services operate across and also to work flexibly to meet the needs of the organisation.
