You can also read more information about University Hospitals of Morecambe Bay visiting our websitehttps://www.uhmb.nhs.uk/ We are very excited to be looking at appointing a District Nurse Band 7 Team Leader to our Out of Hours (OOH) District Nursing Teams as part of the Integrated Community Care Group. Post available:1 x 37.5 hr permanent District Nurse - Team Leader (Band 7). This will be mix of 30 hours OOH and 7.5 hours within core hours. You will have direct and non-face to face contact with patients, their family and carers at each visit or as the need arises to undertake full holistic assessments of health and social needs to co-ordinate and deliver treatment/education/advice and support. Successful applicant will require the ability to travel across the Bay independent of public transport. You will be required to work as an integral part of the Community Nursing Team; flexible working will be required to meet the needs of the patients and will hold 24 hour responsibility for the team caseload. Whilst working with other health care professionals the post holder will develop the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard. As the District Nursing Team Lead you will work closely with the locality senior management team to develop an effective and highly performing team. You will liaise closely with other service providers from both statutory and voluntary agencies on a frequent basis to gather information, advise on additional care needs to ensure appropriate plans are in place. Other key relationships include GPs, hospital staff, care staff, community pharmacists. Such relationships require effective interpersonal skills and communication in identifying the needs accurately, using knowledge and experience in problem solving and identifying solutions to ensure services are co-ordinated and delivered in timely manner appropriate to the patient needs.You will be responsible for managing a small caseload of highly complex patients and working with the staff within your team to ensure high quality assessment and treatment for all.
