The successful applicant will be working at The Red House Surgery, 96 Chesterton Road, Cambridge The post holder will work within their clinical competence as part of a multi-disciplinary team to Provide expertise in clinical medicines management Complete patient structured medication reviews Support patients in the management of their long term conditions Implement systems for safer prescribing Manage repeat and acute prescription requests Manage patients medications on transfer of care Undertake audits using Eclipse The post holder will perform medication reviews for patients with polypharmacy, this maybe in a practice setting or via phone or video consultation. The post holder will also be involved in quality improvement and clinical audit as well as managing some aspects of the Quality and Outcomes Framework (QoF) and Investment and Impact Fund (IlF) work The post holder will work in accordance with practice guidelines and protocols. He or she will be responsible for implementing effective medicine management, identifying areas for improvement and managing change. The post holder will if not already be required and supported to complete the CPPE Clinical Pharmacist training pathway. If you are not already, you will be encourage and supported to become an independent prescriber About us The Clinical Pharmacist will be based at The Red House Surgery in Cambridge. Primary responsibilities are Medication reviews and medicine optimisation Review the ongoing need for each medicine, and supporting patients, ensuring they get the best use of their medicines Provide long term condition clinics Provide telephone medicines support for patients with questions and concerns about their medication Signpost to other services and/or healthcare professionals where appropriate, while working within a scope of practice and limits of competency Support and answer medicine related enquiries from Gp's and other member of the healthcare team Implement practice searches to identify cohorts of patients most likely to be at risk of an unplanned admission To reconcile medicines following discharge from hospital and produce a post discharge care plan Manage systems to ensure continuity of medicines supply to high risk groups of patients Provide medicines information and education to the wider practice team Service development - develop and manage new services that are built around new medicines or NICE guidance Medicines Safety - Identify national and local policy and guidance that affects patient safety through the use of medicines, including MHRA alerts, product withdrawals and emerging evidence from clinical trials CQC - provide leadership to the practice team to ensure the practice is compliant with CQC standards where medicines are involved Any other duties as reasonably required "}
