We can offer a varied working day which utilises many skills to support a range of health care professionals and patients across the organisation. Within our Intermediate Care Services, the role will require the pharmacy technician to: o Undertake medicines reconciliation o speak with patients to support medicines optimisation and adherence o support with discharge planning o address pharmacy-related queries raised by the healthcare team. The role provides the opportunity to work as part of a multidisciplinary team supporting prescribers, nurses, carers and social care team to ensure the safe and effective use of medicines. A sound clinical knowledge is essential for this roles as well as robust IT skills.You will have a broad technical and pharmaceutical knowledge relating to the promotion of high quality, effective and safe use of medicines. Applicants are welcome to apply from all backgrounds of pharmacy. You will receive professional support from our established Medicines Service team based in Hull.We provide a robust induction plan for your new role and identify and support with any training needs. You must have an independent means of transport to travel around the geographic area our services operate across and undertake some home visits. Successful candidates will be able to demonstrate excellent communication and organisational skills and have the confidence to engage and influence a wide range of healthcare professionals.
