About the Role In this pivotal role, you will:* Lead and support a team of Child Health Hub Coordinators , ensuring smooth coordination of services.* Act as the main point of contact for GPs, social care, therapists, nurses, and other MDT professionals , facilitating information-sharing and effective teamwork.* Ensure that referral pathways are clear and efficiently managed to provide the best possible patient care.* Support service improvement by gathering and analysing data, helping to enhance patient engagement and co-production .* Play a crucial part in developing transformational leadership within the team, ensuring continuous professional growth and high-quality service delivery. About us What Were Looking For * Strong leadership and coordination skills, with experience managing a small team.* Excellent communication and relationship-building abilities to work across multiple professional groups.* A proactive and innovative mindset, with a passion for continuous improvement in healthcare services .* Ability to manage and implement policies, oversee recruitment, and provide mentorship and supervision.* Knowledge of Primary Care Networks (PCNs) and child health pathways is desirable but not essential.
