The roles will work collaboratively, within multi professional teams. The qualified ACP will be expected to make complex, autonomous decisions utilising expert practice and knowledge within a supportive team. The trainee roles will be supported while they undertake the academic study and training required to develop into the role. The successful applicant will be required to workautonomously at ACP level working closely with our breast care nursing team and will have an understanding of the family history pathway and undertaking clinics. You will have a clear understanding of the surgical nursing, cancer pathways, and be committed to improving patient experience. You will be committed to the development of advanced clinical practitioner role anddemonstrate evidence of on-going professional and academic development. The qualified ACP band 8a will have a registered MSc in Advanced Clinical Practice or have completed a PG Cert & Dip in Advanced Clinical Practice and is undertaking theirdissertation/learning improvement programme. They will be an established Non Medical Prescriber (NMP) and able to function as an autonomous practitioner across the surgical clinical areas. The job plan has 10%non clinical protected time to achieve all four pillars of advanced practice. The trainee ACP band 7 will be willing to undertake, or is currently registered on an MSc in Advanced Clinical Practice. Consideration will be given to applicants who are have completed the physical assessment module, are registered non-medical prescribers or are completing the NMP course. They will demonstrate a commitment to completing an MSc in Advanced Practice. Funding and 20% non clinical time will be provided to undertake the MSc. About us Detailed job description and main responsibilities Patient Care Delivery Working autonomously - using an advanced level of specialized clinical expertise and judgment in assessment, planning and evaluating care. Communicate effectively with patients and their families in order to assess needs and to provide relevant support, information, education, advice and counselling. Ensure all patients have access to written and verbal information Attend relevant network meetings Collaborate with and refer to other relevant allied health professionals and medical colleagues as required. Offer expert opinion to colleagues and patients in secondary and primary care. Run nurse-led physical and virtual clinics taking full medical history, carrying out clinical examinations. Attend and proactively participate in relevant specialist nurse forums at Trust, Network and national level Effectively liaise with consultant and other members of the multi professional team to achieve a coordinated specialist service.
